A sole proprietorship cannot open an account in the name of the sole proprietorship but can open an account in his own personal capacity. |

- Latest salary slip or Borang J/ EA form (if applicable)

- CDS account opening form (printed out)
- 2 specimen signature cards
- Please include RM 10.00 to open a CDS account
